Contest Brief

The attached file shows an edit screen I am creating for a WordPress genealogy plugin. I am not confident in the design, which is why the contest.

Looking for a modern screen design based on the fields in the attached image. The contest will be judged on the following criteria:

1) Consistency with branding at http://rootspersona.com
2) Convincingly modern design based on current design patterns
3) Judgement on perceived usability

Winner will get the documented reward and a choice to redesign a second tab of the same screen for the same price.
