Contest Brief

I am in need of a 1 page website with a "Signup" form that will email me the contents of the form. I will give fields required. The website is for the promotion and sale of trading software for bitcoin. I have approved images as well as approved wording that I will provide so this should be a fairly simple and quick project. I haveattached the approved marketing PDF. Not all pages need to be on the website, but you can use them as phots or for your content on the page. I want this to be professional and attractive, that is why I am not just using free survey software. By entering you agree to supply me with all code for the website if chosen as the winner. In return I will allow a small banner or logo at the bottom letting people know who designed the website.

Form WIll require to have inputs for Name - Email Address - Username - Street Address

There is a WordPress plugin that does exactly what you want, implementing it is a $20 job. I recommend implementing a captcha as well otherwise you will get blasted with spam. I'm happy to do this for you too. Message me if you're interested.
