While I would like this to be graphically appealing, I think generally icons are not necessary. A couple would be okay. I don't want icons to distract from the content. Thank you all for participating....

I would say for the other questions, leave maybe 3 lines per question for a response

for the questions with sub bullets, they are meant to each be answered as a sub line to the larger questions..for example, #6, the plan, should include Goals, Skills, Resources, Budget, Tactics, Timeline, Measurement (each should have a line in the template) under the #6 heading
