Contest Brief

I need a template designed with a border and background. the colors should include turquoise and gray and it should have a gradient effect to it. i need the border design in both 8.5"x11" and 3.66"x8.5". The following text should be on the bottom of the border: "To sponsor a set in someones zechus: 216.246.7598 / waitingroomcards@gmail.com"
Also the attached logo (tehillim while you wait2) must be included in the design
i attached a word document and a PDF with the basic layout that i am looking for. you are welcome to show me other ideas if you would like.
