Contest Brief

I am looking at having a Loyalty card for Protein Shakes.
It should have 2 rows of 6 boxes with the 6th box being the FREE.
the specs should be as follows:
- 2 sided (from being the logo and website information
- the back being the boxes
- facebook and instagram logos should be on the back somewhere
- colour schemes should include green, black, white. See logos and other marketing material to get a better idea
- print ready with trim lines and 3mm bleed.
- output in multiple formats including png, pdf, ai, eps
- Text on the back should say "Protein Shake Loyalty Card"
- Front of card should have the logo with the "Ultrafit Fitness Centre" wording found on our webpage http://ultrafitfitnesscentre.com.au/

what should be there on the first 5 boxes? Empty boxes or just number them 1-5

Hi, 2 rows of 6 boxes. Does this mean each of the 2 rows will need to have 6 boxes?
Also, what does FREE mean? Should the word FREE be written on last box or should it be left blank?
