I want a logo made out of a picture (and a few words).

I want a vector style logo of a person along the lines of this -

I've attached the pic of the person I want made into the vector logo. Plus the text "Teal's Kitchen"

I need a PSD of PDF file in High quality as the logo will be printed in large scale!
