Contest Brief

Hi, If you are available RIGHT NOW, I need a simple banner header with a contact form. I will give you direction as to how i want it to look and function. But I need this immediately.
I want the styling of the header to look something like thishttps://qodemedia.com/ but maintain branding colors.
Here is the staging site for my new site http://ricka40.sg-host.com/
Text on the left will be "MORE Traffic, MORE Sales" and on the bottom "The Best SEO Company in Toronto"
On the right side we want a contact form (Name, Phone, Email, Website, How Did You Find Us)
You can use the form layouts provided by the theme or a different plugin if you know one that will look better.
All text in the banner must be readable by search engines like the example site provided
I have included the logo and an example of the type of layout we are looking for.
If you complete this quickly, I will have another section for you to do right away.
