Create A Custom 8 1/2'' x 14'' Survey Form

Hello and thank you for looking at my project. I want to create a custom form for our business. It is a combination of several forms that need to be combined and edited into one 8 1/2 x 14 quality word document and pdf. I have attached a sketch of what I want along with all of the remaining documents that will be used to create the one final document. Please go through the files and ask questions as needed.
