Previous Project: I am looking for an extremely SHARP MySQL developer. A person that knows more than how to do your average "SELECT" statement. I am looking for a query that will provide dynamic result headers with corresponding information results. I have attached a txt file to this project, to help better explain what it is I am looking for. I am now looking to add an additional table onto this already created temp table based on the same form ID.

Hi, To be honest with you I am not a great query writer in MySql . However I know the databases throughly and working quite a lot in SQL Server and Oracle .
