Create a sample interactive form for PDF using indesign

Thanks for looking at this project.

I am producing an emag using indesign and want to include a feedback form within the PDF.

At the end of each article I want to have a "feedback icon"

When the reader hovers their mouse over the feedback button (or taps - using an ipad) I would like form to be displayed.

The reader can then complete the form and submit the feedback without leaving the PDF.

The form needs to have the following elements

* Star rating 1 to 5 (one for I did not like the article, 5 been loved the article)

* A free text field for the reader to type a comment

* a submit button

Once submitted it would be good to give the user some feedback the the submit was successful, and the form to hide.

There idea of this project is to produce a nice (but simple) working example so that I can replicate it in my emag.

I can provide a URL to submit the form data to once I understand your requirements, or perhaps submit to a google doc for the example.
