Design half page Advertisement and edit one image

We would like to make a half page advertisement about 4 inches wide, and 10 inches long. Enough to be able to print side by side on a regular sheet of paper. We need a designer who can edit one image and create an advertisement. We are a dental office and the advertisement is geared towards members of the local fitness gym. We have a photo of a beautiful smiling fitness model, however we need you to edit the photo so that her smile is very ugly, with bad teeth. Then we would like the following text "Missing something from your workout?" Then at the bottom we have a coupon for a discounted dental exam and cleaning, with our contact info and included business logo. (Attached) This same advertisement will be copied for another different gym nearby as well, with a different coupon. I need to be able to have a copy and edit the final advertisement as well, in case we change the promotion in the future. Thanks
