Bmi graph for site and banner

I need a graph that can calculate a customer's BMI number. the customer will enter their name, age current weight, height, email address, phone number. This graph will calculate the BMI number and then only send it to the customer's email address. the phone number will be optional if the customer wishes to speak with a consultant.
