Custom search control

I need custom controls build as seen in the image attached. I will need the complete source code and working examples for the same. For examples, you can create your own temp database (which you will need to provide us in order to make sure that the controls do exactly what I want them to). The controls should be usable in [login to view URL]'s latest version (including last two versions). The control should be cross browser compatible and also run on smart phones. I do not want the controls to be built using sliverlight. These should be built using .Net and AJAX. The custom controls should be reusable in other applications by importing the DLL in other applications. Contact me if you need more information.
