Data Entry from PDF to Access DB

I have a total of 2,677 company records in PDF files that I need entered into an MS Access 2002 database. You have two options:
1) If you can build a program to extract or scrape the data from the PDF automatically, that would be perfect! Let me know your price and time frame for developing a program.
2) If you must input this data manually, please provide a price for all 2,677 records and a timeline for completing this work. Please also include a per-record estimate for future data entry jobs.
The data shouldn't be in all caps, type it in standard uppercase/lowercase English. See the attached PDF file for a sample of the data. Each record needs the following 12 fields entered (you can ignore the other fields):
Input Date; Company Name; Address; City; State; Zip Code; First Name; Middle Name; Last Name; Telephone; Company Desc; Filing Date
If needed, I can provide an Access database with the correct fields and a simple data entry screen.

## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
