Basic IE Toolbar

I am looking for a simple custom IE Toolbar that will be used internally so it doesn't need to be real fancy.

I have attached a rough mockup of what I need it to accomplish.

In the screenshot, you will first see a edit box and then a drop down box. The drop down box will need to update the choices based on a configuration

file (XML?) that will be found on our internal Intranet site. When a user clicks the Submit button, it will need to go to a user defined web page

submitting the data from the edit box, drop down combo box and urgent tags as a "GET". It will also need to send the page that the user was on when he

submitted the data.

For example, if a user was on the webpage [login to view URL] and then filled in the edit box with the text: "This is a test of

the edit box", the drop down was selected "Steve" and the urgent was not checked then the user clicked "Submit" it would go to:

http://intranet/[login to view URL]"This is a test of the edit box"&name=Steve&urgent=0&source="[login to view URL]"

The View Open and Create New Buttons will just go to specific pages as defined in the configuration file.

The Toolbar must run on IE 7 on Windows XP and Windows Vista.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
