Adjust Gravity Form Layout CSS PHP Wordpress expert

Hi, I have a gravity form on the following page: [login to view URL] I need to make it to look like this: [login to view URL] The form uses conditional logic such as checkbox disable based on previous selection and checkbox value based on user location so you need to keep those things in mind when you work on it. Make sure this layout is for the required form only and does not effect the other forms.
