Code Form w/ Database Reading and Calculation - Repeat Work Opportunity

I need the coding executed for a form that will be integrated into my [login to view URL] website. Basically, the user will input certain values and the form needs to pull data from a database depending on the values entered. There will also need to be some relatively basic calculations (simple average, If Statements). Please see the excel sheet (represents the database) and the powerpoint slide (represents simple screenshot of form) and the instructions for the data to be read/calculated from the database. Some more things: -This is the first step of developing this form - several iterations expected -Important the the form is visual - graphics in powerpoint are a guide. Good styling skills required. -User entries must be able to be edited from screenshot of Slide 2 of powerpoint -"?" in Slide 2 of form are meant to provide information upon hover -Expect columns in Slide 2 may need to be removed/added. Flexibility important. -The database will be updated on a weekly basis -User needs to be able to "Calculate" with any number of entries (from 1 to 7)
