Custom web form with email output and mySQL table insertion

I have a simple web form with 5 text fields and 1 text area, also with a checkbox, that I have already built. I would like the user to be able to add and remove text fields to the form as needed: Below the 5 text fields is an 'Add More' button, that I would like to create 5 additional text fields when clicked. 5 new texts fields are added every time the 'Add More' button is clicked. Each text field has a 'remove' button next to it. If the button is clicked, the text single field is deleted. The user can create and delete as many text fields as they want. Then, when the form is submitted, I would like two things to happen: 1) All form data is sent via email to a predetermined email address, and the forms data is displayed in a list format in the email body. 2) All form data is stored within a new mysql table record, with all text area values inserted into one DB field, separated by semi-colons. All other values will be stored in their own fields. The 'id' of the currently logged in user is also recorded in a specific field in the new record. All authentication, form coding and DB creation work is complete. I just need the form to interact with the database, and send out an email. Simple. I can provide the existing source files, and DB access.
