Ajax form with javascript, mysql database

I need to have an HTML form that uses Ajax and JavaScript to:
--show a list of filters (on the left side)
--open and close filter details (hide/show link or symbol)
--filter results of the database search, shown on the right side of the screen, according to the checkbox filters selected
The format is shown in the attached html file, which can be used as a starting point. Field names for the database fields are specified in the id for each checkbox and the values in the table are 0,1 (0=does not match this filter, 1=does match this filter). The table is one single table for all fields shown. Returned, based on the filter, are name, link_url, year, pages, description, and these are used in the displayed results.
The filter has to be ajax or some similar means of doing the filtering interactively (showing the results of the filter in the table displayed.) I don't want the user to have to click a button.
