Custom App to post Real-Time data from one system to another

I use Icontact for my email marketing. Every time I upload a list of people, I send them a confirmation message saying "Click here to confirm your email". If the person wants to be a part of my email list, they click it.

Once the person clicks that link, their information automatically get added to my list in Icontact. What I need is for that information to be posted to a lead system called Leads360.

So every time someone clicks the "Click here to confirm your email" link, their information automatically gets posted in my lead system in real time.

I have already talked to Icontact and they said that it is possible to use their API to do functions like that, however I need a custom app created in order to take the information from Icontact and send it to Leads360. I have no idea how this is done, I just know that I need a custom app designed in order to do that.
