Add fixes to new CMS + Data import+ Few bug fixes

PHP, MYSQL, AJAX project This new CMS is copy cat of an old live CMS We need Still Few Fixes: 1. Redo the changes as Shown on screenshots attached 2. Last Processed Column under PI Transport Requests: Please change header column text to: Last Recurring DATE Processed: And Prompt the Dispatchers to Enter Calandar Date The Last Date of the Recurring Day as part OF schedule he is Processing Now *** We do not care for the date he actually Clicked on the date and Time he actually clicked on the Button Proccessd But on the Date he entered in Calander for the Actual recurring Date as Part of Schedule So Ask him before he click Process Button ** We will explain everything if not clear 3. We need to take Data from Old live CMS and update the new PI DB Site so we can have all latest data before we move to use live the new CMS with al fixes you did 4. We need to add a Medical Necessity form Upload if PI provider from NY in addition to Transport Request and Patient Demo You do good job we have lots of work ongoing No flakes and programmers who work on too many project Your bid should include 10% of small additions like column or something but not extra features Nice programmers please Thanks! We release not partial only when all we asked is done So no bid if you need partial release we had programmers got payment milestone and vanished. We would not do this again You have to finish all and after we test we release milestone Thx
