We need someone to take the form on our current site - [login to view URL] and build a new form that utilizes the calender and payment processing form. We currently use a system called VETRAK, it's a client management database. Currently our system on the site takes the form data and imports it into the database. We have the neccesary API's to get the form to talk to the VETRAK database. We need this to be up and running within 3 days with a week total to have it polished and functional! Bid, and good luck!
