adapt existing upload script with progress bar to our needs

**Introduction:**

Our customers often have to send us large files together with some information. Receiving those files via email is not very convenient.

It would be perfect, if our customers could point their browsers to a form on our website, where they chose the file to upload and enter their name, email address, phone number and additional information.

The script would then upload that file, show the customer a progress bar for the upload process, and save it onto the server together with a textfile containing the uploader's name, email, phone number and additional information as previously entered.

We have found an open source script on the internet, that is perfect for that - it just needs to be adapted - this way you don't have to start from zero.

from my knowledge it's the file *[login to view URL]* (a cgi perl script) that is inside the ajax directory manager script that handles the upload and shows the progress bar.

You should create one php file named [login to view URL] or [login to view URL] that displays the upload form together with the progess bar.

Unlike the ajax directory manager there is no login required, nor the other features such as the directories, file preview and so on.

the file upload must be restricted to *.zip and *.rar files.

additionally to the file, the customer must complete required fields, they are as follows:

- email address (required, with basic validation of email address x@y.z)

- full name (required)

- phone number (optional)

- additional information (textarea)(optional)

this customer information will be gathered in a text file that will be saved in the same directory as the customer file and named after the main uploaded file, only with a different suffix: *.txt

the file and folder structure of your script should be as follows:

(this is the upload page containing the form and displaying the progress bar

and upload confirmation once the upload is complete)

(this is the folder in which the uploaded *.rar or *.zip files together with

the textfiles will be saved - again, both files should have the same

filename -except for the suffix

I need this script fast, I want to chose a coder fast, the latest by tuesday morning (19th january) - and then I need the script the latest until wednesday 20th january.**
