credit notes system - PHP/MySql

We need to create a credit notes system, from scratch. We have created CSS mockups, for all pages needed. I have also created all necessary database entries in mysql. Left to do, is PHP Code. We need approx 3-5 Pages. I have created a 6 page document with examples, database info, and exactly what is needed, below is a brief summary. 1) In returns system, underneath all returns info, need section for credit notes. Either need display NO CREDIT NOTES EXIST and link to CREATE/UPDATE CREDIT NOTE. OR, need display CREDIT NOTE SUMMARY and display link to CREATE/UPDATE CREDIT NOTE. 2) CREATE/UPDATE CREDIT NOTE screen is needed next, Here, breaks down to two types of credit notes. will need select date, select from dropdown, add/remove types of credit notes. Need SAVE button, to create database entry. Sorting functions will be needed. The create & Update functions are different, but use the same fields. although same info displayed, there are differences between available fields for create/update. 3) CREDIT NOTE BROWSER - needed to display all created credit notes. need also email function, with counter, for 1st notice, second notice, 3rd notice, etc. Summary of credit note will need to be emailed. NOTE: Other functions will need to be added, for viewing comment history, removing/adding functionality depending on credit note status.
