Conversion of PDF Forms to Adobe Livecycle Forms

We require the conversion of number of multi-page documents (21 documents ranging up to 22 pages each at this point) into Adobe Livecycle documents for the purpose of capturing the data that is entered into the forms for export into a proprietary insurance application built upon an Access or SQL database. We will need to "tweak" the design of some of these forms as well (replacing blank line in from of "yes/no" with a check box). We would also like to be able to develop a script to extract specific pages (those requiring signatures), and present them as a completed pdf to the applicant for printing to be faxed back to us once they have successfully completed and submitted their validated form. The form needs to be validated prior to submission. That is, it cannot be submitted unless all required fields are completed adequately. Also, we would like the user to be able to exit the process part-way through and come back later to finish their work. We are on a very short deadline. Please do not quote on this project unless you are prepared to begin work immediately and are absolutely confident that you are able to fulfill. Also, do not bid unless you can provide 3-5 references that will check out for this type of project. If Adobe Livecycle isn't the right solution, propose something else. We just need the data and a completed PDF to e-fax and/or e-mail to the insurance company. Deadline to delivery is less than 7 days from date of posting. Incidentally, these forms are not as thick as they may sound, in terms of form fields. That is, there aren't a ton of form fields. Lots of instructions...but not a ton of form fields. And some of the apps use the same forms (such as W9), which will cut down on time. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Adobe Livecycle
