Hello. I need to built an amazing "contact form" with the following requirements: - Must have two levels 1) First, the user chooses between 5 car options. 2) Then, the user chooses one of the three motors inside each car option... and also fills the blanks ( Name, Lastname, Email, Phone, Comments) - Must be embedded in an image to get a fancy special effect, something like this: (Form area in red): [login to view URL] This is what I mean with "embedded in an image": [login to view URL] [login to view URL] [login to view URL] - Must be visually engaging, appealing, polished and professional. - Must be able to be seen and used on a website. - Must be able to be seen on a Mobile device (Just to be seen, I´m not asking for a responsive desing). - Must be setup to send the forms to Email. Hola Hector Algunas dudas, la imagen de fondo sera la que has colocado en la descripción o sera otra? Como mencionas 2 niveles, serán 2 imágenes distintas o como? Las opciones de los 5 carros, serán imágenes o un control para seleccionar. Al ir al nivel 2, el paso 1 se oculta, o quieres que todo se vea a la vez? Quieres el formulario en una pagina html normal o para wordpress? saludos Gustavo
