design a contact form that includes 2 buttons -

1. Use design .png of [url removed, login to view] already sent as the design to be inserted into the website. The design colors will need to mirror that of the current theme colors.
2. Create two buttons within the design called:
a. File a Complaint
b. Contact Us
3. Take the current pop form under File a Complaint button located in the header that is green and integrate those fields into the new design so that the integration information from these fields will be sent to posts in pages as it currently is doing now. The design of these fields will need to mirror the design sent in the .png. The current integration is created through Contact Form 7. I would imagine that you would need to pull these same fields in the pop form and integrate them into this new design.
4. Take the current contact us form already created in the sidebar widget and integrate those fields into the Contact Us button to be created in this new design. The integration is already present in the current contact us form so if you complete this form now, the information contained within these fields will be sent to the [url removed, login to view] CRM.
