I just need someone to help me make my paper forms into digital forms.

the forms that i want to create are for the health care industry, i would need these forms to be digital so that I can use them anytime? and anywhere, the forms must have drop down menus with different types of descriptions depending of what is asked? in the form. I? want to be able to fill these forms on line too and save them too online. I also want to be able if i dont finish the forms at work i can do them from my palm pilot or my phone.? whenver you are ready to view the work just let me know and I will sent you the forms that need to be digitalized.
