questionnaire/quiz devlopment

I need to implement a customizable questionnaire in my web application www.athletemonitoring.com. The site works with a mysql database. Athletes and teams are stored in separate tables (team_id and user_id available). I have attached a script containing the structure of the users, teams (tbl_group) and tests tables. Also, I have attached a sample questionnaire (not mine but a one that look and works the same way) The questionnaire must do the following: - Each athlete must be able to fill the questionnaire on a daily basis - An athlete should'nt be able to fill the questionnaire twice the same day, all questions are mandatory - Each question (simple label) is associated with a list of possible answers (radio buttons), each answer includes a description (text string) and a rating (integer) - Once the questionnaire is filled a total score of fatigue is computed based on answer's associated ratings and displayed along with a comment. - I must be able to retrieve score of fatigue and answers for each athlete between 2 dates easily from a query. - We must be must be able to add/delete questions and possible answers - Each questionnaire is associated to a specific team - Each team can have several questionnaires
