Call forwarding software

Hello guys, I’m looking for someone to develop a very simple software that will allow call transfers from my business to other numbers. This is how I am trying to get it to work; First, customers will call my secretary to create an account and tell her five numbers that will be set up in their accounts (each customer will have five different numbers for their account). When they call a specific number the software will detect their caller ID and will ask them to select from the pre-stored numbers the one they want to call using the same phone line (VOIP line). This application must be simple to use and does not require fancy looking. The bid will be a dummy bid of 30$.
