<SHOPIFY> Product Options needed

At our store, we are currently using an Options by BOLD in our product page. We do not wish to use them anymore and have our own.
We need these sets of tasks to be completed:
1. Create multiple options like currently displayed on my website with same exact features except for the conditions below (see attachment below)
2. Must organize/design them in a manner - to our liking (this is a must)
3. Must integrate these options as part of the template so that in the future, we can change or add to our specifications for different products,
