Depending on the complexity of your reporting requirements, two systems may be required. For the data collection, I would recommend using Fastfield by MergeMobile. The forms are customizable, including pre-populated lookup data for providing dropdown choices. We could then create the PDF template for the report, and forward copies via email automatically, if desired. The Fastfield platform provides a configurable, online dashboard to provide data visibility.
If the limits of the Fastfield dashboard do not meet your requirements, the data can be synced with a custom Zoho Creator application . This would allow a greater range of flexibility in the presentation of your data with grouping, photos, etc. as well as providing a customized PDF report.
I recommend using these subscription-based, low code/no code services for this type of project because of the great value gained by having them maintain the function of the data servers, internet connections, database maintenance precesses, etc. that are essential to providing a reliable web-based application. So, there would be an ongoing software fee, payable to the individual company or companies. The actual cost would depend on a number fo factors including user count, roles, platform selection, etc.
