Designs Two Template for Football Team Photos

I need a template made up for team and individual images for a football club. The images are 8 x 10 (20 x 25cm) for individual and 8 x 12 (20 x 30cm) for team photo. I have attached a sample of the photos (not to scale).
The photos are to have the club & sponsor logo (supplied but not high res, will send high res later) as well as a spot on the team photos for the team name & all the names of players. Team colours are green (like in logo) and white.
This is similar to a school class photo.
You will not need to attach the photos to the template just make the template.
Need this to be in .psd.
