Landing Page with WebForm Needed (Code from Scratch - HTML & CSS)

Hi all, I need an experienced web designer who can create landing pages in HTML & CSS from scratch without using any free online landing page creators or website templates. This will be the 1st out of 10 landing pages needed to be created. The successful freelancer who completes the 1st landing page to our satisfaction according to the design template attached below will be awarded the remaining 9 landing page creation and setup projects as well. Scope of Work : 1. Create 1 x Landing Page according to attached PDF design. PDF Design is in rough format (non-PSD), so you will have to create a landing page in HTML & CSS based on the attached PDF design. NOTE THAT NO PSD DESIGNS WILL BE GIVEN TO YOU. 2. Landing Page will have a web form as seen in the attached design, with the web form capable of sending the fill-up details to an email address we'll give you once the user presses the "Submit" button. 3. You will need to embed 4 x Youtube videos per landing page as shown in the attached design, and show thumbnail of each video. 4. Web visitors must be able to click on each of the "Download More Information Below" sections and have the PDF information files downloaded automatically to their desktop. 5. Freelancer will need to upload the landing page to our webhosting account and ensure it appears live online. 6. 100% milestone released upon handover of all project files and completion of above steps. Project cost per landing page creation and setup : US$ 50 Each new landing page project will be awarded to the freelancer only after completion of the previous landing page project.
