We'd like to create a PDF checklist from an Excel sheet that people can download and physically write in (using a pen) names and phone numbers onto it.
It needs to reflect our company branding - we can supply our logo as an EPS file and our colour palate is basically blue, white and black! - and will have the following content -
Front:
Title, logo etc.
Explanatory paragraph or text about the form
24 rows of 7 columns of which the first row contains column titles, the first two columns have text in and the remaining five columns are to be completed by the user (name, work phone, home/mobile phone, work email address and personal email address)
Back:
One sentence of text that explains the back of the sheet is a continuation of front in case the user needs to add more data
