We would like to add a new tab to our facebook with a fill in form which user able to fill in :
1) Name
2) Gender (radio box)
3) Age
4) Drop down menu for selection of category
5) Upload document
6) upload photo
7) DOB
8) Nationality
9) Mailing address
These are some of the field we are looking at for the fill in form for facebook (new tab). Once user click on submit button, it will send the details to a specific email address including the uploaded attachment of document & photo file.
