Create Google Forms and Docs

We like to create about 15 google forms to capture user interview / information - These are in question / answer form to collect information for insurance quotes. The user information will be collected and saved in a spreadsheet in google drive and then like to configure to send the data csv file to few e mail addresses or fill in a pdf document and then e mail. At the time of bidding do this: 1. Send me some examples of the work done by you. or else feel free to send examples of others that you think you can make. 2. As part of deliverable create a 5 minute video on how to do this work and answer questions for learning. 3. Let us know what all good things you can do with the google forms. For example we prefer if the google forms are auto generated with programming (instead of manually creating the forms one question at a time). In other words, as I saw on the google video, to create the form, one has to use the Google Form GUI and create one question at a time. This is time consuming as there may be a lot of questions e.g. 30 to 50 etc. So is there a way to do all this via programming? Please explain any other nice thing that you can do. 4. Remember you are bidding on about 15 google forms that need to be on our google drive. So for this work would you need the user name and password of our e mail address or else you will do it on your own and send me the source code or how? 5. Show us some nice custom looking themes for us to choose / select from 6. We will provide you with the questions and our logo / letter head. What else would you need. Open to ideas or suggestions....
