Debug Flask-Appbuilder Javascript

The demo code is part of a larger application with significant back end. HTML documents are presented to the user and user classification is requested. This input is stored and presented as required. Assistance is required to debug and establish a working front end skeleton. The current demo code built using Flask/Python works more or less. However, it is built requiring routing using Flask routes AND FlaskAppbuilder routes. This creates problems downstream. 1) I want this to work using FlaskAppbuilder routing only. 2) the demo code stores the user input as a separate collection. It should be stored as a property of the document. It should be 4-5 of hours work for someone who is familiar with Flask, Flask-Appbuilder, Javascript, Python, MongoDB. This is part of an ongoing project and future work is likely to enhance this workbench code. The current code base will be shared via GitHub to the selected engineer. The ideal candidate should be able to access Mongo Atlas. Proof is required via a screenshot. If not, will need to setup test data in local MongoDB.
