Simple Javascript Calculator

I need a javascript calculator which does the following:
User enters the amount into "amount" input field.
Javascript calculates a pre-specified percent based on the amount (8% if under 500, 10% if under 1000, 15% if over 1000)
Javascript changes field "daily" to the calculated amount
Javascript also calculates "total" with the daily & the number of days supplied.
