iPhone/Android app: Barcode scan, input related data then email

I need an iPhone/Android app for internal use only that scans a barcode, then lets you input some information (customer, related invoice, date, ...) , shows you the information one last time to confirm, then emails that to a predefined email address.
I know that there are free barcode scanning APIs available so it sounds simple enough in my mind. Just something that connects to that API, then input some information and email it. It doesn't need to be on the App Store. It's for internal use only.
This is what I want, step by step:
1. App launches directly to scan mode
2. User scans barcode
3. App shows scanned barcode along button that says “Programar activación”, that’s "Schedule activation" in Spanish.
4. User hits schedule button then a series of data is prompted: Customer name, product type, plan type, travel dates.
5. Confirmation screen shows scanned barcode and entered data with button to confirm
6. User hits confirm button and app sends email to a predefined email address
