i am working in private company in healthcare, we want want to make an application as follows:
It's like a check list with questions, each question has three scoring criteria ( 0, 1, 2, N/A)
the ones who put the grades ( Evaluators )
some questions has two or more evaluators and the grade with be calculated based on the average of scores. if N/A Not Applicable is given the then score will not be calculated and final grade will not be affected.
any question score below than 2 there must be a an area to put a comment
there must be an option for attachment if the evaluator want to attach a pic or so. and it should be stated the method of evaluation as in attachment. in total we have several but limited methods of evaluation it should be stated.
in general there is major 7 department and the score will be calculated based on the sub questions and will give the final score from 100.
i want each each evalutator has user name and password. and the option to let other companies to download the app and after they register they will get the their own account and they will be able to register 7 evalutators and have their result separately
