File upload script with Admin panel

I need a programmer to create a custom script or to configure an open source upload script of any common web language (PHP, CGI, AJAX, Java) that can be integrated in to an existing website. The script must allow data from a <form> to be included with the file record. Project will include a password protected admin panel to view the form data that is attached to the record of each file, and each file (and form data) must be displayed in order, latest uploads first. This project must include the following features: 1. Script must be executable on the server side 2. Script will be executed from the example page at: [login to view URL] 3. Form fields must be similar to ones on example page 4. All uploads must be routed to my servers, and be configurable 5. Admin panel must have basic file functions, such as edit file name and delete. 6. Must have batch delete feature for all files using radio button. 7. Admin panel must be HTML and include graphics. 8. You must configure this project on my servers. This project cannot use CMS or modules from Wordpress, Joomla, etc., this is a custom programming/configure bid. I must be able to test the final product on your host for 50% payment, I will complete payment when you have made it operational on my server.
