416842 Simple web form modification

I have a squeeze-page-like form on the page: [url removed, login to view], that I would like to modify. Don't worry, it's in Lithuanian, but you don't need to understand anything in order to modify the form.

Where it says "Vardas:", it means "First name:".

And "El. pašto adresas:" means "E-mail:".

I would like to change that form, so that when a "Submit" button is clicked, the form passes on not 2, but 3 parameters to the autoresponder system (we are using Campaign Monitor as an autoresponder, but it could really be anything).

The first 2 variables are entered by the user: first name and email. However, the 3rd variable to be submitted should be called a "modified_name". It should be the same "first name" that a user enters, but with a modified ending. I will give you the rules to modify that name, for example:

1) if the name ends with "-us", replace its ending with "-au";

2) if it ends with "-is", replace it with "-i";

3) if it ends with "-a", leave it as "-a".

The first name with a changed ending will constitute our 3rd variable.

So I just need a quick simple procedure inside the webpage, which would submit a modified name alongside with the first name and the email address. Modified name will only be a variable, not a separate field, so it will not be visible to the webpage user.

Will be looking forward to proposals, and questions, if any.

Thank you.
