Create 1 form and save data in sqlite

Data to be saved in sqlite in my server [I want a flatfile db] .. Create a booking form with fields - name , email , phone number , no. of guests , select table/room , select date and time slot [once selected, it should not be available for others] ,comment box 1 settings page for me - On a php page i should see month dates , yellow in color which have booking click them i see list of that day Note - I should be able to block a day and add time slots. And i should tick to finalize the booking.
