I need a contact form almost exactly like this:

I might need to add or change a couple of the fields.
You will need to use ReCAPTCHA.
Field validation using both Javascript and PHP. On PHP field error, the page should be redisplayed with all entered info pre-populated and a clear indication of the field(s) containing the error. On Javascript field error, the field(s) containing the error should be clearly identified.
The form should be sent by email with a nicely formatted message.
The response page should clearly explain that the form has been submitted and they will receive a response soon.
I would prefer to see an example of the field validation script you will be using. This needs to be done ASAP. I will select a winner in about 12 hours and the project needs to be completed within 12 hours of you being awarded the project.
This must be completed to the end client's satisfaction.
