Zen Cart Amazon Style Checkout + Small Mod Fixes

We're having a hard time with a custom mod on our sites.

such as [url removed, login to view]*[url removed, login to view] (remove star)

Right now the process is as follow

- Add an item to cart

- At cart page, ajax shipping, then when you continue checkout you get

- Guest checkout or create account choice

- Then enter billing information

- then click paypal button

we want to change it to

- Add an item to cart

- At cart page, no ajax shipping

- Amazon Style checkout

[url removed, login to view]

- Enter Billing information page (if new customer account is created there)

- Click on paypal button, billing info passed along to paypal
