simole Web form to Database with Zurmo API

This project is simple. It I only requires a 3 field HTML for that then is submitted to a PHP script that allows the form data to be inserted into a Database. We'll be using the CRM Zurmo. The web form will post directly to the zurmo database using zurmos built in API. I will provide all available documentation for the API as well as full access to the DB, File system, and CRM. This project should only take 2 hours at the most. I NEED IT DONE ASAP!
