I would like every unique user that visits our website to be presented with a pop-up window from our https server. They will be asked to fill out the survey (see attached). The next time they visit the site the window will not pop-up. The survey will reside on a Linux server with PHP and mySQL so it would be nice if you captured the information in nySQL. There is no need to present the result of the information. Contact me if you have any questions.
