Quickform with Smarty template Help

I have started a project for myself to learn web design using LAMP with Pear and Smarty. I have completed 75% of the site but now have questions using QuickForm.

I would like to use QuickForm with my existing Smarty design and need some advice and training.

I have already designed a simple form using Quickform but I am unable to incorporate it correctly into my site which uses Smarty plugins ex. {load_example assign="example"} from the .tpl file and

function smarty_function_load_cc_shipping($params, $smarty) from the .php file

We can start by reviewing my simple QuickForm code.

Make suggestions regarding design.

This will probably result in additional jobs.
