Form to webpage script

I am wanting a form to webpage script made with various options within. I am needing a backend set up for an admin to set up more than one form with various fields the form would be shown on a webpage for a user to fill out so I would also have to have a section for admin to add users that would be able to log into the area to fill out the form(s). Once the user fills out the form the results of the form fields will be displayed on a public webpage within tables and will also be sent by e-mail to the admin or any other user that is listed to send to to e-mail. I will need this script to be set up not only for certain users to log in before submitting but also to have public form as well with the option of the public ones not to be shown on a webpage but only sent to e-mail. If have any questions please ask. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform linux platform running cPanel
